Keeping You in Optimal Health

Our focus is on providing the highest quality care with an interdisciplinary approach to medical services. Rehabilitation Services (physical, occupational and speech therapy) are provided to our residents through our valued partnership with Functional Pathways. Our rehabilitation team in conjunction with the rest of the interdisciplinary team works on development of a comprehensive rehabilitation program to help you meet your goals at your pace. Families can keep track of a loved one’s daily progress in therapy through online access to RightTrack. Functional Pathways' proprietary software RightTrack™ tracks functional outcomes and communicates patient progress to their loved ones, physicians, and facility staff to ensure excellence in patient care. Therapy staff have advanced training in various treatment modalities through a partnership with Accelerated Care Plus (ACP™). ACP's specialized treatment programs combine advanced therapist training with evidence-based clinical protocols and innovative medical technology to provide a higher standard of rehabilitative care.

Our unique Nurse Navigator program provides supportive discharge planning from the day of admission through the day of discharge and beyond. The Nurse Navigator is a Registered Nurse whose primary role is to coordinate all care and services with all your health care providers. The Nurse Navigator will schedule your follow up physician appointments, stay in touch with you after discharge to answer any questions and will work directly with your home care team to ensure that your post discharge care needs are met timely.

We have a full time Respiratory Therapist through our partnership with Respiratory Health Services, a division of Genesis Health Services. Our consultant Pulmonologist provides clinical oversight of our Pulmonary Focused Program in collaboration with the Respiratory Therapist and nursing staff. Our goal is to provide the highest-level quality care to high acuity respiratory patients. Early identification of symptoms paired with quick intervention supports our goal of reducing unnecessary hospitalizations for this high-risk population.
